Enrollment
To enroll and secure a space for your child in our center, a registration form must be filled and the $95.00 registration must be paid at the time of the enrollment. Once your child is read to attend our school the following documents must be in your child’s file.
Health Records:
Form 3040 and 680 or 681 are mandatory for entrance. No student will be allowed to remain in school until a medical form has been filled by the attending physician and it’s on file at the school. These forms should state that the child has had:
